Functional Assessment Form

FIRST: click on the Functional Assessment Form Link.

You will have an extensive set of questions to complete so allow yourself at least 1/2 hour to fill it out to the best of your knowledge

How the Functional Assessment Form works

NEXT:  At the bottom of each section, you have the prompt to add up your responses.  

When you have completed the form and totaled up your responses. Review those number and find what are your top three problem areas.
